If you are interested in surgical or imaging services at Community Animal Hospital, please have your current veterinarian submit your pet's info through our referral portal on our Veterinarians page.

 

Procedure Consent Form

Drop off for anesthetic or sedated procedures is between 7:30 - 8:30 AM the morning of your pet's appointment.

Please make sure that they arrive on an empty stomach. Bring all your pet's medication with you to your appointment. Pets will typically be available for pick up the evening of their appointment. The specific time will be arranged with the clinic on the day of your procedure.

 

Imaging Consent Form

Drop off for ultrasound, echocardiogram, or CT scans is between 7:30-8:30 AM the morning of your pet’s appointment.
Most ultrasounds and echocardiograms are performed without sedation, however, please be prepared for your pet to be sedated if they will not lay still for imaging. For pets getting CT scans, general anesthesia is required. Please make sure they arrive on an empty stomach. Bring all your pet’s medications with you to your appointment.  Pets will typically be available for pick up the evening of their appointment. The specific time will be arranged with the clinic on the day of your procedure. 
Please be aware that for ultrasounds and echocardiograms, your pet will be shaved on the chest or belly to allow for good contact with the imaging probe. 
 
Glucose Curve is an important tool for monitoring diabetes in pets that use insulin. Fill out this form the morning of your pet’s appointment. If your pet has been scheduled for a glucose curve, you should feed your pet and give insulin at home the morning of your appointment. Your pet should be dropped off to the clinic immediately after, so that testing can begin right away. Bring your pet’s insulin and other medications to your appointment. Please expect your pet to stay at the clinic until the evening on the day of their appointment. The specific time will be arranged with the clinic on the day of your visit.
